Cracked Heels
The human foot is a remarkable piece of biological engineering — a structure of
26 bones, 33 joints, and over 100 muscles, tendons, and ligaments that bears the full
weight of the body through every step of life. Yet despite this engineering marvel,
the skin of the foot is surprisingly vulnerable. Cracked heels, medically known as
heel fissures, are one of the most common foot complaints affecting people
worldwide. They range from a minor cosmetic nuisance to a painful, potentially
serious medical condition. Understanding what causes cracked heels, how they
develop, and how best to treat and prevent them is essential for anyone who wants
to keep their feet healthy and comfortable.
What Are Cracked Heels?
Cracked heels occur when the skin around the rim of the heel becomes dry,
thickened, and callused. As the skin loses its elasticity, it can no longer stretch under
the pressure of walking and standing. Instead, it splits — forming fissures that can
vary in depth from superficial surface cracks to deep, painful breaks that may bleed
or become infected. The heels are particularly prone to this condition because the
skin in that area has no oil glands and relies entirely on sweat glands for moisture.
When that moisture is insufficient, the skin dries out rapidly.
The outer layer of skin on the heel — the stratum corneum — naturally thickens
over time as a protective response to friction and pressure. This buildup of dead skin
cells, known as a callus, is the body’s attempt to shield the underlying tissue.
However, when the callus becomes too thick and dry, it loses flexibility. The fat pad
beneath the heel, which acts as a natural shock absorber, pushes outward with each
step. If the surrounding skin cannot expand, it cracks instead.
Common Causes and Risk Factors
A wide range of factors can contribute to the development of cracked heels.
The most fundamental cause is dry skin (xerosis), which can be triggered by
environmental conditions such as low humidity, cold weather, or prolonged
exposure to hot water. Standing for long hours — particularly on hard floors —
places excessive pressure on the heel pad and accelerates skin thickening. Wearing
open-heeled footwear, such as sandals or flip-flops, allows the fat pad to expand
laterally without constraint, increasing the likelihood of fissures.
Obesity is a significant risk factor, as extra body weight increases the
downward force on the heel with every step. Medical conditions such as diabetes,
hypothyroidism, psoriasis, and eczema are closely associated with heel cracking. In
diabetic patients, peripheral neuropathy reduces sensation in the feet, meaning that
fissures may go unnoticed until they become severely infected — a particularly
dangerous complication. Athlete’s foot, a fungal infection, can also damage the
skin’s protective barrier and predispose the heel to cracking. Additionally, nutritional
deficiencies — particularly in zinc, omega-3 fatty acids, and vitamins E and C — can
impair skin integrity and repair.
Age is another important factor. As people grow older, the skin naturally
produces less sebum and becomes thinner, less elastic, and more prone to drying.
This is why cracked heels are especially prevalent among older adults, though the
condition can affect people of any age, including children.
Symptoms and Complications
In mild cases, cracked heels present as dry, flaky skin with a yellowish or
brownish discolouration around the heel rim. The skin may feel rough or hard to the
touch. As the condition progresses, visible fissures appear. Superficial fissures are
often painless but can cause embarrassment and self-consciousness. Deeper
fissures, however, break through into the living layers of skin below, causing pain,
bleeding, and significant discomfort when walking or standing.
If left untreated, deep heel fissures can become infected with bacteria, leading
to cellulitis — a serious skin infection requiring medical treatment. In individuals with
compromised immune systems or poor circulation, such as those with diabetes or
peripheral artery disease, infected fissures can lead to ulceration and, in severe
cases, may necessitate medical intervention. This underscores the importance of
treating cracked heels promptly rather than dismissing them as a trivial cosmetic
issue.
Treatment Approaches
The cornerstone of cracked heel treatment is consistent moisturisation. Thick,
emollient creams containing ingredients such as urea (typically in concentrations of
10-25%), lactic acid, glycerin, or shea butter are highly effective at rehydrating and
softening hardened skin. These products work best when applied immediately after
bathing, when the skin is still slightly damp and the pores are open. Wearing cotton
socks after application helps lock in moisture overnight — a technique often called
‘sock therapy.’
Before moisturising, it is beneficial to gently exfoliate the thickened skin using a
pumice stone or foot file. This removes the outermost layer of dead cells and allows
the moisturiser to penetrate more effectively. However, overly aggressive scrubbing
should be avoided, as it can worsen irritation. Soaking the feet in warm (not hot)
water for 10-15 minutes before exfoliating softens the callus and makes the process
easier and safer.
For deeper or infected fissures, a visit to a podiatrist is advisable. Podiatrists
can debride (professionally remove) hardened skin, apply liquid bandage or
medical-grade adhesives to seal deep cracks, and address any underlying
biomechanical issues that may be contributing to the problem. In some cases,
custom orthotics can redistribute pressure away from vulnerable heel areas.
Prevention
Prevention is far easier than treatment. Maintaining a regular moisturising
routine, even when the heels appear healthy, goes a long way toward preventing
fissures from developing in the first place. Choosing well-fitting, supportive
footwear with a closed heel and adequate cushioning helps protect the skin from
excessive pressure and lateral expansion. Staying hydrated by drinking sufficient
water supports skin health from the inside out.
Addressing underlying medical conditions — such as managing blood sugar
levels in diabetics or treating skin conditions like psoriasis — is equally important. A
balanced diet rich in essential fatty acids, antioxidants, and vitamins supports
healthy skin regeneration. Regular foot inspections, particularly for those with
reduced sensation, help catch problems early before they escalate.
Cracked heels are a widespread yet often overlooked condition that can
significantly affect comfort, mobility, and quality of life. While they may begin as a
dry skin problem, they can escalate into painful, potentially serious medical
complications if ignored. Fortunately, with consistent care, appropriate
moisturisation, sensible footwear choices, and timely medical attention when
needed, cracked heels are both preventable and treatable. The feet carry us through
life — taking a few minutes each day to care for them is a small investment with
significant returns for overall health and wellbeing.
