condition affecting the skin between the toes. Characterised by the softening,
whitening, and breakdown of the interdigital skin due to prolonged moisture
exposure, it represents a significant source of patient discomfort and, when left
untreated, a potential gateway to more serious secondary infections. Understanding
the aetiology, clinical presentation, and evidence-based treatment strategies for this
condition is essential for podiatrists, general practitioners, and allied health
professionals alike.

Aetiology and Pathophysiology
The interdigital spaces of the foot are uniquely predisposed to moisture
accumulation. The close approximation of the toes, combined with occlusive
footwear and the natural perspiration of the plantar skin, creates a warm, moist
environment that disrupts the skin’s protective barrier. The stratum corneum — the
outermost layer of the epidermis — becomes over-hydrated, resulting in maceration:
a process whereby the skin softens, wrinkles, and loses its structural integrity.
Contributing factors include hyperhidrosis (excessive sweating), diabetes mellitus,
peripheral vascular disease, immunosuppression, obesity, and the prolonged
wearing of tight-fitting or non-breathable footwear. Athletes and individuals who
spend extended periods on their feet are particularly susceptible. The fourth
interdigital space — between the fourth and fifth toes — is the most commonly
affected site, owing to its naturally tighter configuration.
Macerated skin provides an ideal substrate for microbial colonisation.
Gram-negative bacteria such as Pseudomonas aeruginosa and Proteus mirabilis are
frequently implicated, as are dermatophyte fungi, most notably Trichophyton rubrum
and Trichophyton interdigitale. The condition commonly known as ‘athlete’s foot’
(tinea pedis) often coexists with or precipitates interdigital maceration, creating a
mutually reinforcing cycle of skin breakdown and infection.

Clinical Presentation and Assessment
Clinically, interdigital maceration presents as pale, soggy, and often malodorous skin
between the toes. The affected tissue may appear white or grey and can peel away
easily. Patients frequently report pruritus (itching), a burning sensation, and general
discomfort during ambulation. In more advanced cases, fissuring and ulceration may
develop, raising the risk of cellulitis and deeper soft tissue infection — a concern of
particular gravity in immunocompromised individuals and those with diabetic foot
disease.
Thorough clinical assessment should include a detailed history of footwear habits,
occupational exposure, hygiene practices, and any underlying systemic conditions.
Microbiological swabbing of the interdigital space can help identify causative
organisms and guide antimicrobial therapy, particularly in persistent or recurrent
cases.
Treatment Strategies
Management of interdigital maceration is largely centred on moisture reduction,
restoration of the skin barrier, treatment of any secondary infection, and patient
education to prevent recurrence. A staged, multimodal approach typically yields the
best outcomes.

Drying and Hygiene Measures
The cornerstone of treatment is meticulous drying of the interdigital spaces after
washing. Patients should be advised to gently but thoroughly dry between the toes
using a clean towel or, where skin is fragile, a hairdryer on a cool setting held at a
safe distance. Foot soaks are generally contraindicated during active maceration, as
they exacerbate moisture retention.
Drying agents such as aluminium chloride solution (10-20%) can be applied topically
to reduce hyperhidrosis. Talcum powder and medicated foot powders containing
absorbent agents like zinc oxide are useful adjuncts to help control interdigital
moisture throughout the day. Toe separators — made from foam or silicone — can
be effective in holding the interdigital spaces apart, improving airflow and reducing
skin-on-skin friction.

Antimicrobial and Antifungal Treatment
When fungal infection is identified or strongly suspected, topical antifungal agents
form the primary pharmacological intervention. Azole-based preparations (such as
clotrimazole or miconazole) and allylamines (such as terbinafine) are widely used
first-line agents, typically applied twice daily for two to four weeks. These agents
disrupt fungal cell membrane integrity and have demonstrated high efficacy against
dermatophyte infections in clinical trials.
In cases of confirmed bacterial superinfection, topical antibiotics such as fusidic
acid or mupirocin may be employed. For widespread or resistant infections,
systemic therapy may be warranted: oral terbinafine or fluconazole for persistent
fungal infection, and systemic antibiotics guided by culture and sensitivity results for
bacterial involvement. Antiseptic solutions, including dilute sodium hypochlorite
(Milton solution) or potassium permanganate foot soaks at low concentrations, can
assist in reducing bacterial load during acute flares, though care must be taken to
avoid over-drying healthy surrounding tissue.

Debridement and Wound Care
In cases where maceration has progressed to tissue breakdown, gentle debridement
of devitalised or hyperkeratotic skin by a podiatrist is indicated. This facilitates
topical agent penetration and promotes healthy tissue regeneration. Any fissures
should be dressed with appropriate low-adherence wound dressings and monitored
closely, particularly in high-risk patient populations.
Footwear and Lifestyle Modification
Patient education regarding appropriate footwear is a vital component of both
treatment and long-term prevention. Shoes made from breathable natural materials
such as leather or mesh fabrics are preferable. Moisture-wicking socks — ideally
composed of merino wool or synthetic performance fibres rather than cotton —
should be recommended, and socks should be changed at least daily, or more
frequently if significant perspiration is present. Going barefoot in communal areas
such as swimming pools and gym showers should be discouraged to minimise
re-infection risk.
